Using Fink’s Self Directed Guide for Significant Learning, I have created a Learning Outcome 3 Column Table for one of the courses (physical, mental, social) in the online wellness blended learning initiative. No matter what health challenges a person may face, they have to change their mindset to achieve wellness. Reflecting on the benefits of adopting a healthy lifestyle can push the learner to take full advantage of the wellness initiative and build the healthy lifestyle needed to achieve the life they need. Using Fink’s guide, I have created my 3 column table. The Learning Outcomes 3 Column Table includes the foundation, applying knowledge, integration,  human dimension, and learning how to learn (Fink). It is my wish to help the learners to create a significant blended learning environment where they can achieve the wellness goals that they've set for themselves.

The course outcomes planning process and the 3 column table will influence my innovation plan because it will give my initiative some direction and more of a purpose. The planning process will help me and the learners to be organized as they work towards and meet the wellness goals that they are creating. The BHAG for this initiative is thatThe learner will adopt healthy lifestyle changes to enhance their life through health education, health resources, guided health discussions, and health support.
